Coconino County Retirees – FY19 Benefit Open Enrollment

Remember that this year’s Benefit Open Enrollment starts April 30, 2018 and ends May 14, 2018!

All paper or emailed forms must be received in HR by 5 pm on May 14th.

If you are making any changes during this Benefit Open Enrollment, please complete and return the FY18 Retiree Open Enrollment Change Form to the mailing address below or email to acontreras@coconino.az.gov.

If you have life insurance as a retiree, we ask that you complete and return the Beneficiary Form to the mailing address below or email to acontreras@coconino.az.gov:
