Coconino County Retirees – FY18 Benefit Open Enrollment

Remember that this year’s Benefit Open Enrollment starts May 1, 2017 and ends May 15, 2017!

All paper or emailed forms must be received in HR by 5 pm on May 15th.

If you are making any changes during this Benefit Open Enrollment, please complete and return the FY18 Retiree Open Enrollment Change Form to the mailing address below or email to mjohn@coconino.az.gov.

If you have life insurance as a retiree, we ask that you complete and return the Beneficiary Form to the mailing address below or email to mjohn@coconino.az.gov:
