If you are a testing or vaccine provider, please complete the following survey so we may better disseminate information on available locations

All vaccine locations offer vaccines for all adults and children 6 months of age and older unless indicated otherwise.
The vaccine locations offer all types of vaccines and boosters. Contact the vaccination site for the latest information.
Please bring your COVID-19 vaccination card with you if seeking a 2nd or booster dose.
Masks are recommended at all vaccination locations.
Children 6 months -17 years of age must be accompanied by a guardian.

At-Home COVID-19 Tests

Information for at-home COVID-19 tests: www.COVIDTests.gov

If you took an at-home COVID-19 test and tested positive, please do not call to report the case to Coconino County Health and Human Services. Instead, please inform the people you have had close contact with of your positive case. A close contact is anyone who was within 6 feet of an infected person for a total of 15 minutes or more over a 24-hour period. Monitor symptoms and contact your healthcare provider if they worsen.
